Job Description:
The Case Manager ensures veterans and their families receive essential supportive services to achieve personal stability and employment goals. This includes developing individualized service plans, coordinating access to critical resources such as housing, transportation, and healthcare, and providing ongoing support throughout each veteran’s journey. The ideal candidate is compassionate, organized, and driven to help others succeed.
Key Responsibilities:
Consult with program applicants to assess needs and determine eligibility for services
Create individualized employment and service plans with measurable goals
Provide consistent case management, evaluation, and support to veterans in need
Coordinate access to community resources including housing, transportation, and healthcare
Maintain accurate documentation and ensure compliance with Department of Labor and agency standards
Develop and sustain partnerships with community organizations to expand service outreach
Attend relevant meetings, outreach events, and collaborative initiatives to promote program success
Job Requirements:
Bachelor’s Degree in Social Work, Psychology, Human Services, or related field preferred (or equivalent experience)
Minimum 3 years of experience in social work, nonprofit, or human services setting
Experience working with veterans or homeless populations strongly preferred
Compassionate, empathetic, and dedicated to helping others
Strong communication, problem-solving, and organizational skills
Maintains confidentiality and professionalism in all interactions
Works well independently and collaboratively within a team environment
Valid driver’s license with reliable, registered, and insured vehicle
