Case Manager Job Description:
The Case Manager provides trauma-informed advocacy and comprehensive case management services to veterans and their families experiencing or at risk of housing instability. This role is responsible for guiding participants through the process of securing and maintaining stable housing while connecting them to critical resources and supportive services.
The ideal candidate is compassionate, resourceful, and highly organized, with the ability to build trusting relationships and empower clients to achieve their goals. Working with a caseload of approximately 15–30 veteran households, the Case Manager serves as a dedicated advocate who helps coordinate services, navigate benefits, and remove barriers to housing stability.
Key Responsibilities:
- Manage a caseload of veteran households and provide ongoing case management services utilizing a Housing First approach.
- Conduct assessments to determine eligibility, identify needs, and connect clients to appropriate services and resources.
- Collaborate with clients to develop individualized housing stability plans with measurable goals and outcomes.
- Provide ongoing support, advocacy, crisis intervention, and resource navigation services.
- Coordinate with internal teams, community providers, housing resources, and benefit programs to support successful client outcomes.
- Conduct outreach activities to identify and engage veterans in need of supportive services.
- Assist eligible households with accessing financial assistance and other supportive resources.
- Maintain accurate case records, documentation, and compliance with grant, regulatory, and agency requirements.
- Build and maintain strong relationships with referral sources, community stakeholders, and veteran service organizations.
- Participate in community meetings, agency events, and interdisciplinary team discussions to support program goals.
Case Manager Job Requirements:
Education & Experience- Bachelor's degree or equivalent work experience in Social Services, Human Services, Nonprofit Administration, Case Management, or a related field.
- Minimum of two (2) years of experience in nonprofit, social services, housing services, case management, or related program delivery.
- Experience working with grant-funded programs, veteran services, housing programs, or low-income populations is highly preferred.
Knowledge & Skills
- Strong case management, client advocacy, and resource coordination skills.
- Excellent communication, relationship-building, and interpersonal skills.
- Ability to assess client needs, develop service plans, and support participants in achieving goals.
- Strong organizational and time-management abilities with the capacity to manage multiple priorities.
- Critical thinking and problem-solving skills with the ability to navigate complex situations and identify effective solutions.
- Experience maintaining accurate documentation and ensuring compliance with program and grant requirements.
- Proficiency with Microsoft Office applications, including Word, Excel, and Outlook.
Additional Requirements
- Valid, unrestricted driver's license with an acceptable driving record.
- Current vehicle registration and automobile insurance.
- Ability to travel within the community to meet with clients, service providers, landlords, and community partners.
