Diabetes Health Program
Pay: $22.00 per hour
Schedule: Monday - Friday, 10:00 AM – 2:00 PM
Position Summary
Are you passionate about community health and making a real difference? We’re looking for a Community Health Worker (CHW) Canvasser to support a two‑year, community‑based diabetes health initiative focused on outreach, education, and connection to care. In this role, you’ll engage residents through door‑to‑door canvassing, community events, and neighborhood outreach to promote diabetes prevention, self‑management, and access to health and social services. The ideal candidate has 1–2 years of community outreach or CHW experience, a strong connection to the communities served, and a genuine desire to build trust and improve health outcomes.
Key Responsibilities
- Conduct door‑to‑door canvassing and community outreach to identify individuals at risk for or living with diabetes
- Provide culturally appropriate education on diabetes prevention, management, nutrition, physical activity, and medication adherence
- Screen individuals for basic health and social needs using approved tools
- Connect community members to healthcare providers, diabetes education programs, and social support services
- Build and maintain trusting relationships with residents, community partners, and local organizations
- Participate in health fairs, workshops, and community events
- Document outreach activities, referrals, and participant interactions accurately and on time
- Collaborate with program staff, nurses, and care teams to support follow‑up
- Assist with data collection and reporting for grant outcomes
- Adhere to confidentiality, HIPAA, and ethical standards at all times
Required Qualifications
- 1–2 years of experience as a Community Health Worker, canvasser, outreach worker, or in a similar community‑based role
- Strong knowledge of—or lived experience with—community health issues
- Interest in diabetes prevention, management, and health equity
- Excellent interpersonal, communication, and relationship‑building skills
- Ability to work independently in the field and collaboratively as part of a team
- Comfortable with door‑to‑door outreach and engaging diverse populations
- Basic computer skills and ability to document activities electronically
- High School Diploma or GED required
