Schedule: Monday-Friday, 8:30am-4:30pm
Pay Rate: $24/hour

Case Manager Job Description:
- Manage a caseload of veteran households and provide ongoing case management services.
- Assess eligibility, identify service needs, and develop individualized housing stability plans.
- Connect veterans and families to housing resources, VA benefits, public assistance programs, and community services.
- Coordinate services with internal teams and community partners to achieve successful outcomes.
- Maintain accurate, compliant case documentation and ensure adherence to program regulations and agency policies.
- Conduct outreach activities and cultivate referral partnerships within the community.
- Assist eligible households with financial assistance programs and resource navigation.
- Participate in program, community, and stakeholder meetings as needed.
Case Manager Job Requirements:
Required Qualifications:
- Bachelor's degree or equivalent experience in Social Services, Human Services, Nonprofit Management, Case Management, or a related field.
- Minimum of two (2) years of experience in social services, nonprofit programs, case management, or related human services work.
- Experience working with diverse populations and low-income households; veteran services experience is highly valued.
- Strong organizational, communication, time management, and interpersonal skills.
- Proficiency with Microsoft Office applications and case management databases.
- Valid driver's license, reliable transportation, current vehicle registration, and automobile insurance.
