Pay Rate: $24.00/hour

Job Description:
- Conduct comprehensive client assessments and identify barriers to stability and success.
- Develop, implement, and monitor individualized service plans.
- Connect clients to housing, employment, healthcare, behavioral health, financial assistance, and other supportive services.
- Provide advocacy, crisis intervention, and ongoing case management support.
- Maintain accurate and timely case notes, documentation, and program records.
- Collaborate with community agencies, government organizations, and service providers to coordinate services.
- Monitor client progress and adjust service plans as needed to ensure successful outcomes.
- Ensure compliance with program requirements, agency policies, and funding guidelines.
Job Requirements:- Bachelor's degree in Social Work, Human Services, Psychology, Counseling, or a related field preferred.
- Minimum of 2 years of experience in case management, social services, housing services, or a related field preferred.
- Experience working with veterans, individuals experiencing homelessness, or underserved populations is highly desirable.
- Strong knowledge of community resources and social service systems.
- Excellent communication, relationship-building, and problem-solving skills.
- Ability to manage multiple cases while maintaining detailed and accurate records.
- Proficiency with Microsoft Office and electronic case management systems.
- Ability to work independently and collaboratively within a team environment.
- Valid driver's license and reliable transportation preferred.
