Job Description
The Team Lead Canvasser plays a key leadership role in a Community Health Worker (CHW) Campaign focused on diabetes prevention, management, and connection to care. This position combines hands‑on door‑to‑door outreach with mentoring, coaching, and coordination of a team of CHW Canvassers.
The Team Lead serves as a model for best practices in community engagement while supporting team members in building trust, reducing barriers to care, and delivering culturally appropriate health education. This role is ideal for an experienced canvasser or outreach professional ready to guide others and drive high‑quality community impact.
Key responsibilities include:
- Serving as a lead canvasser while mentoring and supporting a team of CHW Canvassers in the field
- Conducting door‑to‑door outreach related to diabetes prevention and management
- Providing culturally appropriate education on diabetes, nutrition, physical activity, and medication adherence
- Coaching team members on engagement strategies, messaging, and relationship‑building
- Assisting with onboarding, shadowing, and ongoing coaching of canvassing staff
- Monitoring daily outreach efforts and supporting accurate documentation and data collection
- Participating in health fairs, workshops, and community events alongside the team
- Collaborating with program staff, nurses, and care teams to support participant follow‑up
- Upholding confidentiality, HIPAA, and ethical standards at all times
Job Requirements
- 2+ years of experience in canvassing, community outreach, or community health work
- Previous experience in a lead, mentor, or supervisory role preferred
- Strong understanding of community health issues and health equity
- Excellent communication, organization, and leadership skills
- Ability to work independently in the field and effectively support a team
- Comfort with door‑to‑door outreach and engaging diverse populations
- Basic computer skills and ability to document activities electronically
- High School Diploma or GED required
