Overview:
The Senior Case Manager serves as a key resource within the our veteran housing program, providing advanced case management services to Veterans and their families experiencing or at risk of homelessness. The position supports complex or high-barrier cases, ensures quality and consistency in service delivery, and assists in mentoring and guiding other case managers. The Senior Case Manager collaborates with internal and external partners to achieve rapid housing placement, housing stability, and increased self-sufficiency for program participants.
This role requires a high level of clinical judgement, through knowledge of community resources, and the ability to balance direct service responsibilities with leadership and support of the broader case management team.
Senior Case Managers are responsible for maintaining a caseload of veteran clients for the program. Key processes include:
- Providing ongoing support and expertise through assessment, planning, implementation, and overall evaluation of the veteran’s needs as drawn from his/her goal/action plans.
- Obtaining information contributing to the client’s situation and evaluating these, along with the client’s capabilities/skills.
- Submission of State Rental Assistance Program and TBRA – Homes Voucher Applications.
- Collaborating weekly with the program, Supportive Service to Veteran Families (SSVF), and other community programs on action/service plans, coordinating approaches for successful outcomes.
- Following up to determine quantity and quality of service provided to the veterans and the status of his/her care
- Participate in coordinated entry and case conference meetings to support intra-agency and interagency collaboration and continuity of care.
- Mentor and provide informal leadership to junior case managers; serve as a subject matter resource for complex cases.
- Provide support during staff onboarding and training as assigned by the Program Manager.
Responsibilities:
- Report directly to the Program Manager.
- Review and ensure client eligibility and exits are completed according to program timelines.
- Assist in approving or preparing documentation for Temporary Financial Assistance (TFA) Requests when needed.
- Support quality assurance and file review process to promote compliance and high service standards
- Represent our Program at all community meetings including Case Conferences and By Name Lists.
- Develop effective plans for clients, in coordination with all partners.
- Provide Case Management with a focus on “housing first”
- Connect clients to VA resources/benefits
- Connect clients to mainstream resources/benefits
- Support and execute fundraising events
Skills/Competencies:
- Computer Skills: Including but not limited to Microsoft Office Suite and Adobe.
