Overview:
Case Managers provide Case Management for approximately 15-30 veteran households at a time, supporting development and achievement of individual housing stability plans.
- Consulting with potential clients to assess needs and determine program eligibility
- Working with clients to create individualized housing stability plans with client-specific goals.
- Monitoring clients from the time of intake until they exit the program
- Ensuring that clients obtain needed supportive services
Case Managers are responsible for maintaining a caseload of veteran clients for the Housing program. Key processes include:
- Providing ongoing support and expertise through assessment, planning, implementation, and overall evaluation of the veteran’s needs as drawn from his/her goal/action plans.
- Obtaining information contributing to the client’s situation and evaluating these, along with the client’s capabilities/skills.
- Submission of State Rental Assistance Program and TBRA – Homes Voucher Applications.
- Collaborating weekly with the BVH, Supportive Service to Veteran Families (SSVF), and other community programs on action/service plans, coordinating approaches for successful outcomes.
- Following up to determine quantity and quality of service provided to the veterans and the status of his/her car
70% of the Case Manager’s work effort should be to case management and approximately 30% with outreach.
Responsibilities:
- Report directly to the Program Manager, in coordination with Senior Case Manager
- Conduct outreach to identify and provide services for target populations
- Develop effective plans for clients, in coordination with all partners.
- Provide Case Management with a focus on “housing first”
- Connect clients to VA resources/benefits
- Connect clients to mainstream resources/benefits
- Support and execute fundraising events
- Perform duties as directed
Client Satisfaction:
- Assist clients, family members or other clients with concern and empathy; respect their confidentiality and privacy and communicate with them in a courteous and respectful manner.
- Answer and refer telephone calls or other inquires to ensure accurate and timely communications are facilitated.
- Identify yourself in a pleasant and positive manner.
- Case management meetings.
- Coordinate, through case management, an active behavioral health treatment plan when appropriate.
- Provides educational presentations related to functional life skills, time management, anger management, communication skills, values clarification, budgeting, independent living and parenting skills.
Skills/Competencies:
- Computer Skills: Including but not limited to Microsoft Office Suite and Adobe.
- Other required skills: The ability to communicate with all levels of the organization. This position also requires a solid, self-directed individual who can excel both independently and as a team player in a fast-paced environment. Candidates must be able to complete tasks at accepted levels of quality.
