Requirements
Bachelor’s degree in a health-related field preferred, with applicable experience
Valid driver’s license, reliable transportation, and current auto insurance required
Knowledge of Medicare and Medicaid regulations required
Responsibilities
Coordinate Medicaid (MA) and Medicare benefits with participants, families, and governing agencies
Maintain accurate and up-to-date electronic case management records, including:
Annual MA recertifications
Skilled Nursing Facility (SNF) Day 3 submissions
Invoicing and documentation tracking
Complete thorough documentation of all interactions, including:
Communicate participant status changes promptly to interdisciplinary team members
Provide administrative assistance to participants, including help with applications, forms, and accessing community resources
Assist participants in completing Medical Assistance (MA) recertification applications, ensuring all required documentation is submitted within designated timeframes to maintain financial eligibility
Keep participants and their families informed about:
