Case Manager Job Details:
Schedule: Monday-Friday, 8:30 AM-4:30 PM
Pay Rate: Starting at $24.00 per hour

Job Description:
- Manage a caseload of veteran households utilizing a Housing First case management approach.
- Conduct client assessments to determine eligibility, identify needs, and connect veterans to appropriate services and resources.
- Develop individualized housing stability plans with measurable goals and actionable steps.
- Provide ongoing support, advocacy, crisis intervention, and resource navigation services.
- Coordinate with internal teams, community organizations, landlords, and service providers to support successful outcomes.
- Assist eligible households with accessing financial assistance and supportive resources.
- Conduct outreach efforts to identify and engage veterans in need of services.
- Maintain accurate case documentation and ensure compliance with program, grant, and agency requirements.
- Build and maintain referral relationships with community partners and veteran-serving organizations.
- Participate in agency meetings, community events, and collaborative initiatives that support program success.
Job Requirements:
Education & Experience
- Bachelor's degree or equivalent work experience in Social Services, Human Services, Case Management, Nonprofit Administration, or a related field.
- Minimum of two (2) years of experience in case management, housing services, social services, nonprofit programs, or related fields.
- Experience working with veterans, low-income populations, housing programs, or grant-funded programs is highly preferred.
Knowledge & Skills
- Strong case management, advocacy, and resource coordination skills.
- Excellent communication and relationship-building abilities.
- Ability to assess client needs and develop effective service plans.
- Strong organizational and time-management skills with the ability to manage multiple priorities and deadlines.
- Sound judgment, critical thinking, and problem-solving capabilities.
- Experience maintaining accurate documentation and ensuring compliance with program standards.
- Proficiency in Microsoft Office, including Word, Excel, and Outlook.
- Ability to work independently while contributing positively to a collaborative team environment.
Additional Requirements
- Valid, unrestricted driver's license with an acceptable driving record.
- Current vehicle registration and automobile insurance.
- Ability to travel within the community to meet with clients, landlords, service providers, and community partners.
